By accepting this agreement, I certify that everyone I am getting a ticket for: is not currently known to be infected with the COVID-19 virus, is not currently waiting for the results of a COVID-19 test, and is not currently caring for anyone with COVID-19. I also certify that everyone I am getting a ticket for does not have any of the following conditions:
*fever of 100.4 or higher
*new onset of moderate to severe headache
*shortness of breath
*new cough
*new sore throat
*vomiting and diarrhea
*new loss of sense of taste or smell
*fatigue from an unknown cause
*muscle or body aches
